Please note: 

i. Items marked with a red asterisk “＊” must be filled out, without which, you will not be able to proceed to the next step.  

ii. All items must be carefully filled in and double check before submission. Once submitted, no item can be corrected. 

iii. After submission, please sign on the printed application form. The printed papers include 3 pages with page 1 and 2 are the visa application form and the bar code is on page 3, which is for the Centre’s further processing. All the 3 pages must be brought or posted to the Centre.
